Our Fall season tryouts are Aug. 19-21 for cross country, football, soccer, and volleyball.
Welcome to Fall Season 2019!
All PIL high schools have now moved to an online sports registration program through Family ID. This is the new way to register your child for athletics at Madison High School. To register for fall 2019 teams, please use this link.
This takes the place of all paper forms, but a copy of each student’s physical must still be uploaded during online registration, or a hard copy turned in to Athletics in A 27B before tryouts on August 19. If you have used Family ID to register for youth sports, you do not need a new account, just be sure your physical is current (they last 2 years).
Check with Mr. Kelleher or Mrs. Graves if you have questions about how to register for a team. Cross country and football will take all interested students, but soccer and volleyball hold tryouts for space on the teams. Please be sure to attend all days of tryouts, or make arrangements directly with the coaches (scroll to coach contact info below).

Madison Athletics is happy to announce Jeremy Sutter as our new head men’s soccer coach! Coach Sutter has been a part of the Madison Soccer since 2015 as an assistant coach. Current assistant coach Adam Stone will return next fall as well, and will be joined by Keith Vidos and Ben Grosscup to round out our staff. Welcome all!

Welcome to Madison Athletics. On our site you'll find resources necessary to compete for Madison High School. All players must be cleared BEFORE the first day of practice with a physical or update form, guidelines and policies contract, and a blue card present in the Athletics Office. You'll also find contact information for our coaches. Go Senators!
1. Madison registers athletes through Family ID at https://www.familyid.com/madison-high-school-2 You must upload or turn in a current sports PHYSICAL as part of the registration process. The online form takes the place of blue cards, guidelines and policies forms and update forms, and is easily renewed each season. 2. A Physical Examination Form MUST be on file in the Athletic Office (pick up forms in A-27 B, or main office). The Physical Examination Form must be dated within two calendar years of the end of the season. A link to this form can be found under the Registration Tab above. Or by clicking the "physical" hyperlink. IF YOU ARE NOT REGISTERED ONLINE, you must turn in the following with your physical. ALL forms go to the athletics office, not to coaches. 2. Medical History Update Form. Students with physicals older than one calendar year from listed Start Date must fill out a Medical History Update Form. Students with Physical Examination Forms dated within the last calendar year DO NOT have to complete this form. A link to these forms can be found above under the registration tab, or by clicking "Medical History" hyperlink. Please turn Medical History Update form into the athletic office. 3. Code of Conduct Form. Signatures are required from both athlete and parent/guardian. This only needs to be turned in once each year and is good for all sports during the school year. A link to this form can be found above under registration tab, or by clicking "physical" hyperlink.. Please turn Code of Conduct Form into the athletic office. 4. Athletes also need a Blue Emergency Card; A new Blue Card must be filled out for each sport during the school year. Signature is required from parent/guardian. You cannot participate in a tryout or practice unless the coach has your Blue Card in hand. They are not available online but can be found in the Main Office or on the desk outside the Athletic Office. Turn the Blue Emergency Card into the athletic office. 5. Pay the PIL Participation Fee after tryouts conclude for cut sports. The fee is $200 or $35 per sport. Pay online through SchoolPay at www.schoolpay.com/link/sportsfees If paying in cash or writing a check, please mark down student-athlete name and sport on memo section of check. Make check payable to Madison HS and turn into the Bookkeeper in the main office.
